Transitioning home after hospitalization or managing chronic conditions often requires ongoing skilled medical care that goes beyond what family members can provide. You may need these services following hospital discharge when continued medical care is necessary, for chronic conditions requiring regular nursing oversight, when mobility makes clinic visits difficult, or when doctor-ordered treatments can be safely administered at home.
Medical home health care is provided by licensed nurses, physical therapists, occupational therapists, and other healthcare professionals who deliver skilled medical services in the home setting. This care is typically ordered by a physician and may be covered by Medicare or insurance when medically necessary.
Services include skilled nursing care, wound care and infection management, medication administration and management, physical and occupational therapy, chronic disease monitoring, post-surgical care, and patient and caregiver education.
